If you would like to know the cost of your next visit with us, please ask the Front Desk staff, send a message through the Patient portal or request one next time you call to schedule an appointment. MFHC will provide an estimate in a timely manner.

If your actual charges are more than $400 above the estimate, you can initiate a provider-patient dispute resolution process at www.cms.gov/nosurprises/consumer-protections/Payment-disagreements.com
Starting a dispute resolution process will not reduce the quality of health services you receive at MFHC.
