Stephen Glass, DDS, Dental Director

Dr. Glass’s interest in dentistry for underserved areas began when he saw the value that a nonprofit clinic brought to remote populations in his home state of Virginia.  Ever since, he has been passionate about dentistry and community service. Dr. Glass and his wife came west after he completed dental school at Virginia Commonwealth University. Along with their dog Oso, they enjoy being in the mountains, climbing, skiing, and biking.

Cathy Lettice, RDH

Cathy Lettice is originally from Buffalo, New York, where she received her training at SUNY Erie Community College. She became a Registered Dental Hygienist in 2006 and enjoys keeping her skills and knowledge up-to-date through continuing education courses. Cathy moved to the mountains of Colorado in 2012 with her family and joined Mountain Family in 2016. She is licensed in both New York and Colorado, and recently received her permit from the State of Colorado to place Interim Therapeutic Restorations in small cavities. She enjoys running, hiking and life in the mountains.

Connor Rivers, DMD

Dr. Rivers is the dentist for our newly expanded Basalt Integrated Health Center. He is passionate about restoring dental health, using a wide range of dentistry practices to achieve it. In his free time, he enjoys University of Kansas basketball, snowboarding, mountain biking and enjoying everything the Roaring Fork Valley has to offer. Dr. Rivers earned his Doctor of Dental Medicine degree from the Midwestern University College of Dental Medicine and completed a one-year hospital residency in general dentistry at Truman Medical Centers in Kansas City, Missouri.

Sanju Basi, DDS

Dr. Sanju Basi is dedicated to providing excellence in dentistry. Born and raised in Toronto, Canada, Dr. Basi attended Western University in Ontario for his undergraduate studies. He pursued his passion for dentistry at the University of Detroit Mercy in Michigan, where he earned his Doctor of Dental Surgery degree in 2018. Prior to graduation, Dr. Basi had the great fortune to visit Colorado’s Western Slope and fell in love with the area and its people. He is proud to be able to serve our local population and joined Mountain Family Health Centers in 2018.

Anai Quezada-Bencomo, RDH

Anai joined Mountain Family in 2016, returning to the community where she grew up. Her focus is on education, preventive care and motivating patients towards better oral health. Fluent in both English and Spanish, Anai enjoys building relationships with patients from all of the diverse communities of the Western Slope. She is a graduate of Colorado Northwestern Community College. In her spare time, she enjoys reading, exercising, outdoor activities, and spending time with her family.

Audrey Schaiberger Fisher, DDS

Audrey Fisher joined Mountain Family in 2018 and is dedicated to providing compassionate, individualized dental care. She is originally from New Castle and is overjoyed to be back with family, friends, and the wonderful community here.  In her free time, she loves skiing, swimming, hiking, and relaxing with a good book. Dr. Fisher earned her Bachelor of Science from the University of Colorado Boulder in 2008 and then continued on to earn her Doctor of Dental Surgery degree from the University of Colorado Denver in 2018.

Courtney Schwind, DDS

Courtney started practicing dentistry in 2014 at the University of Texas Health Science Center San Antonio, where she later earned her Doctor of Dental Surgery in 2016. Courtney worked in private practice dentistry, then made the transition to work in public health in 2018 and joined Mountain Family. Prior to becoming a dentist, Courtney earned her Bachelor of Science, with a digital photography minor, from Texas State University in San Marcos, Texas. Courtney was born and raised in the Lone Star State, lived in Wisconsin/Minnesota for two years after finishing dental school, and now resides with her family in Colorado. Courtney is a competitive/sponsored ultra mountain trail runner and also enjoys snowboarding, skiing, rock climbing, and sharing her passions with her husband and son.

Charlene Pariera, RDH

Charlene Pariera is part of our CO MDI project with the Delta Dental Foundation of Colorado. She works with our Glenwood Springs health center to expand dental education and services to patients not already receiving dental care. Charlene graduated from Oregon Institute of Technology, where she earned her Bachelor of Science in Dental Hygiene. Charlene is the first member of her family to graduate from college and moved to Colorado in 2014. She moved to La Junta Colorado to become part of the Colorado Medical Dental Integration (CO MDI) Project with the Delta Dental of Colorado Foundation. Charlene has worked in private practice as well as public health. In December of 2018 Charlene moved to Glenwood Springs to work for Mountain Family Health Centers. She is happy to be working with Delta Dental on the CO MDI project again. Charlene likes to hike, camp, raft, bike, snowshoe enjoy hot springs. Family is very important to her with three daughters and three granddaughters.

Estela Lujan-Ortiz, RDH

Estela Lujan-Ortiz joined Mountain Family in 2019. She is originally from Glenwood Springs and grew up in Garden City, Kansas. Estela earned her Bachelor of Science in Dental Hygiene in 2017 from Wichita State University and is fluent in Spanish and English. Estela is motivated to help improve community health by providing exceptional care to patients and spreading awareness of dental health. In her free time, she enjoys having fun with her daughter and friends.

Before joining Mountain Family in 2000, Jenny served several years in the United States Peace Corps at clinics in Yemen, then at a mission in Guatemala, and then at a Free Standing Birth Center in the Bronx, New York City, where she developed her midwife birthing skills. She holds a Certified Nurse Midwife degree from the University of Pennsylvania, a Family Nurse Practitioner degree from the University of Colorado, and a Bachelor of Science in Nursing from the University of Rochester. Jenny contributes time at Mountain Valley Developmental Center, and she enjoys exploring Colorado’s natural wonders with her husband and two daughters.

Dr. Percy enjoys the broad spectrum of challenges that family medicine offers, although he has particular interests in pediatrics and preventative medicine. While he grew up in Cortez, Colorado and completed his medical residency in Salt Lake City, it was a rotation at Mountain Family that opened his eyes to the attractions of the Colorado and Roaring Fork Valleys, and he’s been here ever since. Dr. Percy earned his MD degree from the University of Colorado School of Medicine, after earning a Bachelor of Science in Biology, with honors, from Stanford University. He is working to improve his moderate Spanish. When not at work, you can find Dr. Percy with his wife (a pediatrician herself) and three kids, exploring the outdoors on foot, bike or skis, or traveling the world.

Dr. Heckert joined Mountain Family in 2014 after completing her residency. She attended Philadelphia College of Osteopathic Medicine in Philadelphia, PA, and grew up in Delaware. As an osteopathic physician, she is trained in manipulation to treat a variety of musculoskeletal and internal conditions–a discipline that fit with her long interest in exercise science and nutrition. After hours, Dr. Heckert enjoys skiing, running, rock climbing, and learning Spanish.

Zackary Kent, MD, Edwards Site Medical Director

Dr. Kent joined Mountain Family in 2019. Previously he was an Assistant Professor of Family Medicine in the Air Force, where he taught for nine years. Upon completion of residency training in 2009, Dr. Kent completed additional fellowship training in obstetrics and then began teaching at the University of Nebraska – Offutt Air Force Base Family Medicine program. Four years later, Dr. Kent then joined the faculty of the Travis Air Force Base Family Medicine Residency program near Fairfield, California, where he completed the remaining five years of his Air Force service. Dr. Kent has interests in women’s health and obstetrics, in-office procedures, as well as family and community health. He is board certified in Family Medicine by the American Academy of Family Physicians and is excited to be a part of the Mountain Family team.

Kent Petrie, MD

Dr. Petrie has practiced medicine in the Vail Valley since 1979, and was previously recognized as Colorado Family Physician of the Year. Dr. Petrie earned his medical degree from the University of Virginia in 1976 and started the Eagle Care Clinic for underserved patients in Eagle County, in 1994. He was excited to continue practicing medicine at the clinic when Mountain Family assumed ownership in 2014. Dr. Petrie is certified by the American Board of Family Medicine, and also sees patients at Vail Valley Medical Center and is the Eagle County Health Officer. Dr. Petrie travels abroad on medical missions, enjoys being a grandfather, and is an active elder at Eagle River Presbyterian Church.

Chris Tonozzi, MD, Director of Data Quality

A native of Glenwood Springs, Dr. Chris Tonozzi is a family physician and a self-proclaimed “data nerd.” Dr. Tonozzi was Mountain Family’s first physician on the Western Slope. He returned to his home town of Glenwood after seeing the need for affordable health care for low income residents. He earned his degree in medicine from the University of Colorado in 1991. Dr. Tonozzi has a special interest in pediatrics and travel medicine. He is also passionate about digitizing medical care and making sure Mountain Family is at the forefront of data-driven health care. He is board-certified in Family Medicine and also sees patients at Valley View Hospital. Dr. Tonozzi loves to spend time traveling and being outdoors with his family, skiing, kayaking and trail running.

Sandy Deveny, MD

Sandy grew up in an Air Force family, traveling the globe, but she always considered Colorado home. Sandy was thrilled to join Mountain Family in 2008, moving from Akron, Ohio. Prior to that she had worked in various emergency departments and urgent care facilities in Ohio. She also worked with Dr. B.S. Bonyo in his inner-city office and was instrumental in developing a health clinic in Dr. Bonyo’s Kenyan village. Sandy is co-founder of Bonyo’s Kenya Mission, a 501c3 dedicated to ongoing support of the clinic and a growing child educational sponsorship program. Sandy attended University of Texas in Austin for her undergraduate degree, competing in cross country and track. She then completed her medical degree at The Medical College of Ohio and residency in emergency medicine at St. Vincent Medical Center/The Toledo Hospital. She is board certified in Emergency Medicine and primarily does acute care at Mountain Family. Sandy is also a designated civil surgeon approved to carry out immigration medical examinations. When not working, Sandy enjoys all the activities the area has to offer.

Maria Y. Chansky, MD

Dr. Maria Y. Chansky joins Mountain Family Health Centers in October 2018. Dr. Chansky earned her MD in 1995 from the University of Massachusetts Medical School in Worcester, Massachusetts and graduated from Harvard University in 1989 with a Bachelor of Arts, Cum Laude, in Biological Anthropology. Dr. Chansky has extensive experience practicing medicine in Colorado, New Zealand, and New England. She is a member of the American Academy of Family Physicians, the Colorado Academy of Family Physicians, the Physicians Committee for Responsible Medicine, the Wilderness Medical Society, and the American College of Sports Medicine. Dr. Chansky is board certified with the American Board of Family Medicine as well as for Advanced Cardiovascular Life Support, Pediatric Life Support, and Neonatal Resuscitation. Dr. Chansky is also a Fellow of the American Academy of Family Physicians. She enjoys many hobbies, including playing guitar & ukulele (badly but enthusiastically), basketball, and mountain biking

Gail Mizner, MD

Dr. Mizner loves her role as the internal medicine consultant for MFHC, focusing on autoimmune diseases, endocrinology, and HIV care. Before returning home to Colorado in 2010,  she practiced internal medicine, hospice, and HIV care for six years in Prescott, AZ. Before that, she practiced internal medicine in Northern Chile for seven years, running HIV programs in two hospitals, becoming fluent in Spanish along the way. She also spent two years in Ely, Nevada, as the only internist in an extremely rural and isolated setting. She is a former faculty member in internal medicine at the University of Colorado Health Sciences Center, as well as former head of physician education for UCHSC’s AIDS Education and Training Center. Dr. Mizner earned her MD from the Yale University School of Medicine. When not in the clinic, she can be found with her family on local slopes and hiking trails.

Rachel Hickman, FNP

Rachel Hickman has been practicing as a nurse practitioner since 2016.  She earned her Master of Science in Nursing from University of Colorado at Colorado Springs, where she also received her Bachelor of Science in Nursing in 2012. Rachel has been practicing as a Family Nurse Practioner since 2016 and as a nurse since 2002. Rachel has three children and has lived in the Eagle River Valley for seven years. She enjoys outdoor activities such as mountain biking, cross country skiing, alpine touring and reading a good book.

Casey Aguirre, DO

A Salt Lake City native and fluent Spanish speaker, Dr. Aguirre brings a passion for manipulation, preventative medicine and sports medicine, including the education of young athletes and coaches in concussion risks. Dr. Aguirre attended Lake Erie College of Osteopathic Medicine and earned his board certification in family medicine at Southern Colorado Family Medicine in Pueblo, Colorado. Outside of medicine, he is a dedicated family man, outdoor enthusiast and avid baseball fan.

Claire Graff, PA‑C

Claire joined Mountain Family in 2015, after earning her Master of Physician Assistant degree from Wichita State University–a program that emphasized primary care in rural and under-served areas. Claire sees patients primarily at the Basalt clinic, where she works to help our community achieve lower rates of lifestyle-related diseases, while she also works on her skills in Spanish. Her background includes guiding on Colorado rivers and trails, as well as owning her own business marketing outdoor recreation to visitors. She enjoys spending time with her husband and son and most weekends you’ll find her surfing river waves or working on a side project that promotes women in river sports.

Mary Patterson, FNP

Before coming to Mountain Family in 2016, Mary spent years working with cancer patients and the underprivileged in Memphis, Tennessee. She received her Master of Science in Nursing from the University of Missouri-Barnes College of Nursing in 2003, and is a 1996 graduate in nursing from the University of Tennessee. On weekends, you’ll find Mary and her family exploring all of the fantastic outdoor activities that Colorado has to offer.

Susan Orcutt, FNP

Susan Orcutt joined Mountain Family in 2008 bringing her eagerness to care for our medically underserved community members. She has a specific passion for the care of women and strongly believes that promoting the health of women is imperative to the well-being of our families and our entire community. Susan received her Master of Science in Nursing and Family Nurse Practitioner degree from the University of Colorado Health Sciences Center in 2007. She is a member of the American Association of Nurse Practitioners and the Colorado Society of Advanced Practice Nurses, and also oversees clinical family medicine and nurse practitioner students at Mountain Family Health Centers. Susan is a Colorado native who enjoys spending time with family outdoors.

Natasha Ellwood, PA-C

Natasha joined Mountain Family in 2015, and works in family medicine, preventative care and management of chronic disease. She also brings a background in musculoskeletal complaints and sports medicine. She is a 2013 graduate from Midwestern University’s Physician Assistant program in Phoenix, Arizona. A native Coloradan, Natasha enjoys running, camping and hiking, as well as traveling and knitting.

Emily Borkovec started with Mountain Family Health Centers in 2017.  She is originally from Iowa, and received her undergraduate degree at Minnesota State University. Emily attended the Physician Assistant program at Red Rocks in Denver. Emily’s background is in community health, and she previously worked at two other Federally Qualified Health Centers in Colorado. Emily is passionate about the underserved having access to quality, affordable health care. In her free time, she enjoys hiking, backpacking, mountain biking, cooking, and spending time with family and friends.

Diane Purse, PNP

Diane’s passion is working with infants, children and families. She has been a champion for Eagle County’s youngest, most vulnerable residents for more than 15 years. Diane’s professional dedication  focuses on childhood  dental hygiene, preventative care and childhood literacy. She earned her Pediatric Nurse Practitioner degree from State University of New York at Stony Brook  in 1992. When Diane isn’t tirelessly caring for the next generation she enjoys reading, trail running and traveling.

Debra "Deb" Burns, ANP-C, GNP

A skilled Adult Nurse Practitioner, with a well-established talent for the listening and diagnosing that field entails, Debra also specializes in geriatric care. After work, she enjoys outdoor adventures with her husband and two sons, and she plays a wicked game of ping-pong. Debra earned her MSN degree from the University Colorado Health Sciences Center in 1992, with a dual Nurse Practitioner Adult/Geriatric specialty, and she graduated Magna Cum Laude from the University of New Mexico with a BSN in 1986.

Adriana "Melissa" Meyer, FNP

Melissa Meyer graduated as a Family Nurse Practitioner in December 2017 from Southern Adventist University near Chattanooga, Tennessee. Previously she was a labor and delivery nurse for five years. Melissa also earned her undergraduate degree Southern Adventist University. Melissa and her husband, Will, relocated to Colorado from Chattanooga, and enjoy doing anything outdoors. They are excited to live in Colorado and have two big dogs, a German shepherd and a husky.

Corrinne Johnson, PA-C

Corrinne came to Mountain Family in 2015 from Salud Family Health Centers in Fort Collins. Corrinne enjoys seeing patients of all ages, helping them achieve better health and wellness. She speaks some Spanish and is always working on getting better. She fell for Colorado while working summers in Rocky Mountain National Park as she attended Penn State University in her native Pennsylvania. Corrinne moved to Colorado in 2002 to work for Salud Family Health Centers in Estes Park for a few years before returning to school, first at the Red Rocks Physician Assistant program, then earning her master’s degree from Saint Francis University. Corrinne’s hobbies include hiking, biking, skiing, camping and backpacking with her husband.

Kerrie Kuhl, CNM

Kerrie is a Certified Nurse Midwife and sees patients at our Avon and Eagle Family Planning clinics with our Title X program. Kerrie earned her Master of Science in Nursing in 1997 from the University of Colorado Health Science Center in Denver and her Bachelor of Science in Nursing in 1990 from the University of North Dakota – Grand Forks. Kerrie joined Mountain Family in 2018 and enjoys snowshoeing and skiing when she’s not working.


Gary Schreiner, PhD, LMFT, Behavioral Health Director

Dr. Gary Schreiner brings three decades of experience in private practice and institutional settings, including extensive experience with adolescents and the U.S. military, counseling servicemen and their families around the world. Previously he was also a school counselor and football and baseball coach. Dr. Schreiner holds a PhD in Marriage and Family Therapy from Texas Tech University. A native Coloradan from Boulder, he chose life on the Western Slope for its slower pace and outdoor opportunities. Dr. Schreiner and his wife enjoy skiing, golf, scuba and travel.

Barbra Corcoran, MSW

Barbra Corcoran joined Mountain Family as a Behavioral Health Counselor in 2015. A longtime resident, Barb earned her Master of Social Work degree from the University of Denver Graduate School of Social Work in 2015, where she was recognized as a Western Colorado Area Health Professions Scholar and received the Dean Catherine F. Alter award. After hours, you’ll usually find Barbra with her husband and their four children.

Melvin F. Betancourt, LCSW

Melvin Betancourt was born and raised in Guaynabo City, Puerto Rico. He graduated with a Bachelor’s degree in Psychology from La Universidad de Puerto Rico in 2002, and earned his Master’s in Clinical Social Work from the University of Central Florida in 2007. Melvin joined Mountain Family Health Centers in 2016, and brings his experience working with children, families and adults with mental health/behavioral problems. Melvin appreciates working in a team setting and making a difference in the community. He enjoys traveling, sports, hiking, and live music.

Matthew "Matt" King, MA, MFTC, CACII

Matthew “Matt” King started his practice at a community mental health agency in 2015 in Denver and has worked in both in-patient and integrated health care settings. Matt has worked extensively with teens, adults, and couples with special areas of expertise including trauma (PTSD), Marriage & Family Therapy, and substance abuse. Matt works in a collaborative style that allows him to join with the client in a safe, validating, and accepting way. Matt’s treatment modality is integrative and includes Dialectical Behavioral Therapy, Existential Psychotherapy, Pragmatic Experiential Therapy with Couples, and Eye Movement Desensitization and Reprocessing (EMDR). Matt earned his Master of Arts in Marriage and Family Therapy in 2016 from Regis University. He is an avid mountain biker, hiker, and hot springs soaker.

Sara Jacobs, LCSW

Sara Jacobs is a big believer in Mountain Family’s integrated approach to health care and solution-focused, strengths-based social work. Based at our Basalt clinic, she came to us from Mind Springs Health, where she has long experience in crisis assessment, individual therapy and case management for adults and seniors. Sara holds a Master of Social Work degree from the University of Maryland. A Portland, Oregon, native, Sara has called the Roaring Fork Valley home since 2010. After hours, you’ll often find her cooking, skiing, and walking dogs at Aspen Animal Shelter.

Oyen Hoffman, MS, LMFT, LAC

Oyen Hoffman joined Mountain Family in 2016, and is a Licensed Marriage and Family Therapist and a Licensed Addiction Counselor. He is passionate about helping his patients and their families. Oyen earned his Master in Psychology with a Marriage and Family Therapy emphasis from Chapman University in 2012. He is also a Master Addiction Counselor (MAC) with the National Association for Addiction Professionals and is certified to treat Post Traumatic Stress Disorder (PTSD) by the International Association of Trauma Professionals and the National Center for PTSD. Oyen enjoys spending time with his family.

Geneane Lazo, BS, MA, NCC, LPCC

Geneane fell in love with Colorado in 2016, while on vacation, and decided to move here. She relocated from Baton Rouge, Louisiana in January 2018. Geneane received her Bachelor of Science in 2009, in Communication and Science Disorders, from Southeastern Louisiana University. She graduated with Honors (Gamma Chapter of Chi Sigma Iota- Counseling Honor Society) with her Master of Arts in Clinical Mental Health Counseling from Nicholls State University in Thibodaux, Louisiana in 2017. Geneane enjoys soccer, volunteering as a youth leader in her church, and other activities.

Candice joined Mountain Family in 2018. Raised in Chicago’s western suburbs, she graduated with a Bachelor of Arts in Psychology from DePaul University in 2008 and earned her Master of Arts in Clinical Mental Health Counseling from Lewis University in Romeoville, Illinois in 2014. Candice has experience working with children, adolescents and adults in a wide range of behavioral health settings. Candice is excited to be a part of Mountain Family and is looking forward to Colorado’s year-round activities.

Christina Andrade, BS, MSW

Cristina is a Roaring Fork Valley local. She joined Mountain Family in 2019, after finishing an internship with us. Cristina earned her Bachelor’s Degree in Social Work in 2018 from Colorado Mesa University and her Master in Social Work from the University of Denver in 2019. Cristina’s experience is in school social work and behavioral health services in an integrated care setting. She is passionate about solution-focused, trauma-informed care, as well as providing services in Spanish. On her free time you might see Cristina on the trail while hiking, biking, at the gym or traveling the world.

Laine Morgan has been practicing as a psychiatric nurse practitioner since 2017. Laine earned her Master of Science with a specialty in psychiatry from the University of Colorado; she obtained a Bachelor of Science in Nursing from Regis University in 2010. Laine is passionate about working with substance abuse, trauma, mood disorders, anxiety, developmental disabilities, and more. She was born and raised in Colorado, and in her spare time she enjoys the outdoors, watching hockey, or curling up with a good book.

Jennifer Girard, DBH, LPC, LAC

Jennifer Girard is a graduate of the Doctor of Behavioral Health online program with Arizona State University (2016) and since returned to ASU as an Academic Associate. Jennifer received her Master of Arts in Clinical Mental Health Counseling from Adams State University in 2013 and is a licensed professional counselor as well as a licensed addiction counselor. Jennifer has significant experience with inpatient and outpatient substance use treatment, as well as crisis intervention. Jennifer provided behavioral health services in an integrated primary care clinic for the homeless with the Stout Street Health Center with the Colorado Coalition for the Homeless in Denver and served with the cystic fibrosis team at Children’s Hospital Colorado. Working in community health in an integrated setting with underserved populations is Jennifer’s passion. She is an Eagle Valley native and enjoys traveling and photography in her spare time.

Casandra Armas, BS, MSW

Casandra Armas was born and raised in Eagle County. She earned her Bachelor’s Degree in Social Work in 2018 from Colorado Mesa University. In 2019, Casandra received her Master of Social Work from the University of Denver. She joined Mountain Family in 2019, shortly after finishing her internship with us. Casandra is passionate about working with children and teenagers. She finds the brain and human behavior to be fascinating and enjoys everything about the behavioral health field. In her spare time, you might see Casandra volunteering, traveling, playing soccer, or watching hockey.
