The providers at Mountain Family Health Centers bring years of specialized training and experience to our practice. Our nurses and providers actively participate in continuing education programs and continually strive to stay current with health trends and practices. In addition, they are involved in the communities we serve by conducting health fairs and are involved with various other non-profit organizations. Our Community Health Center is one of the most technical in the state and our staff utilizes Electronic Medical Records to monitor and track our patient’s health and well being. AMY RYN, DO, CMO
Chief Medical Officer
Dr. Ryn is a recent addition to Mountain Family Health Centers serving as a family physician as well as our Chief Medical Officer. She graduated from Midwestern University (Arizona College of Osteopathic Medicine) in 2001. Dr. Ryn has been practicing medicine since 2004 and is currently board certified in family practice and holistic/integrative medicine. She established her own practice in Arizona and introduced an integrated care model to the area. She was an officer in the Air Force and completed her active duty time in 2013. Dr. Ryn comes to us from a community health center in Prescott, Arizona. She is very excited to be in Colorado and part of our team!
CHRIS TONOZZI, MD
Director of Data Quality
Dr. Tonozzi has been with Mountain Family Health Centers since 1999. He earned his MD at Colorado School of Medicine and practiced at the University of Colorado Department of Family Medicine. Dr. Tonozzi is fluent in Spanish and is responsible for making our clinic one of the leading community health centers in the state of Colorado. Dr. Tonozzi is currently the Director of Data Quality. Mountain Family Health Centers values Dr. Tonozzi’s efforts in adopting Electronic Health Records and technical computerization of our clinic. When Dr. Tonozzi is not in the office you’ll find him on the slopes and rivers enjoying time with his family.
JENNY LANG-BURNS, FNP, CNM
Site Medical Director
Jenny has been with Mountain Family Health Centers since 2000 after graduating from the University of Colorado’s Family Nurse Practitioner program. Previously, Jenny completed her Certified Nursing degree at the University of Rochester and finished her Certified Nurse Midwife degree at the University of Pennsylvania. Jenny served several years in the United States Peace Corps in Yemen practicing at health clinics in this poverty stricken area. It was during her stint at an Ecumenical Mission in Guatemala that Jenny found her passion working with mothers and children. Jenny went on to serve at a Free Standing Birth Center in the Bronx, New York developing her midwife birthing skills. Jenny moved to Glenwood Springs, Colorado, in 1995 and now enjoys practicing part-time at Mountain Family Health Center as well as serving as the Glenwood Springs Site Medical Director. Jenny enjoys spending time with her husband and two daughters experiencing the wonders of the Colorado Mountains.
SUSAN ORCUTT, FNP
Mountain Family Health Centers welcomed Susan Orcutt to our staff in July of 2008. Susan graduated from the University of Colorado Health Sciences Center’s School of Nursing in Denver, Colorado, in May of 2007 with a Master of Science in Nursing, Family Nurse Practitioner degree. She received her Bachelor of Science in Nursing from the University of Colorado Health Sciences Center’s School of Nursing and graduated Magna Cum Laude in 2003, following completion of her pre-nursing requirements at University of Colorado in Boulder. Susan brings valuable experience from Grand River Medical Center in Rifle, University of Colorado Hospital in Denver, and Boulder Valley Women’s Health Center. “I’m looking forward to promoting health and wellness in our community,” said Susan. Susan is a Colorado native and when she is not caring for patients in the clinic, she can be found enjoying time outdoors with her family skiing and backpacking in the mountains of Colorado and Utah.
SANDY DEVENY, MD
Dr. Deveny came to Mountain Family Health Centers from Akron, Ohio. She practiced at the MEDGroup as a staff physician specializing in urgent care and occupational medicine after spending 12 years in emergency medicine. Dr. Deveny also worked with B.S. Bonyo, D.O. at his inner-city office and was instrumental in developing a health center in Kenya with Dr. Bonyo. She also volunteered at the Open M free clinic in Akron. Dr. Deveny graduated from The Medical College of Ohio at Toledo, and earned her residency from St. Vincent Medical Center / The Toledo Hospital with Board certification in Emergency Medicine. She is certified to practice in both Ohio and Colorado. Dr. Deveny has also traveled and practiced medicine in Peru, and she as co-founded Care Kenya and practiced at the medical clinic in Kenya, Africa. Sandy enjoys spending time with her family and dogs, and enjoys running, hiking, biking, skiing, photography and cooking.
ANNELIESE HECKERT, DO
Dr. Anneliese Heckert joined Mountain Family Health Centers in 2014 after completing her residency. She attended Philadelphia College of Osteopathic Medicine in Philadelphia, PA, and grew up in Delaware. Anneliese studied exercise science in college, and nutrition and physical activity promotion is close to her heart. She is an osteopathic physician who enjoys practicing manipulation for a variety of musculoskeletal and internal conditions. Anneliese enjoys skiing, running, and rock climbing in her spare time, therefore is loving Glenwood Springs. Although she is not fluent in Spanish, she is learning, so please be patient!
KATY RIEVES, MD
Dr. Katy Rieves graduated from Bowdoin College in Brunswick, Maine in 1988 with a History degree. After college she was eager to live in Seattle, Washington and accepted a position there as a Medical Assistant, beginning her journey in health care. She moved to her hometown of Oklahoma City, Oklahoma to start a Masters of Public Health program in 1991.Deciding she missed patient care, she applied to medical school. After graduating from University of Oklahoma Medical College in 1996, she completed a Family Medicine Residency at Mayo Clinic in Scottsdale, Arizona in 1999. She moved to Grand Junction, Colorado in 1999to work with Mercedes Cameron, M.D. at The Women’s Place, a women’s health clinic designed to integrate multiple styles of healing. In 2002 she started her solo private practice in Grand Junction, which focused on a holistic approach to patient care. Wanting to work with an underserved population, she started work in Rangely, Colorado in 2013 and joined Mountain Family Health Care in May 2016. She enjoys being a medical detective and deciphering complicated cases as well as helping patients make behavior changes for a healthier lifestyle. When she is not working, she enjoys mountain biking, hiking, and skiing.
MARY PATTERSON, FNP
Mary Patterson came to Mountain Family Health Centers in February, 2016 from Memphis, Tennessee. Mary graduated from the University of Tennessee in Knoxville in May, 1996 with her nursing degree. She received her Masters of Science in Nursing from the University of Missouri-Barnes College of Nursing in 2003. Before moving to Colorado, Mary spent many years serving the medically underprivileged in a primary care setting in Memphis. She also has had years of experience with oncology patients. Mary and her family recently relocated to Glenwood Springs and is enjoying all the outdoor activities that this area has to offer.
Internal Medicine Consultant
Dr. Mizner earned her BA in French Literature at Amherst College and her MD at the Yale University School of Medicine. She returned home to Colorado to do her residency in internal medicine at the University of Colorado Health Sciences Center and then stayed on at UCHSC as a faculty member in the General Internal Medicine Division and the Head of Physician Education for the AIDS Education and Training Center. In 1995, Dr. Mizner and her husband and two sons moved to Ely, Nevada where she enjoyed practicing as the only internist in an extremely rural and isolated setting. In 1997, the family moved again to Northern Chile. After becoming fluent in Spanish and taking 6 exams to revalidate her medical degree, she practiced internal medicine and ran HIV programs in two different Chilean public hospitals. After returning to the States in 2004, Dr. Mizner did internal medicine, hospice, and HIV care in Prescott, AZ. When the family decided to move to the Roaring Fork Valley in 2010,”it felt like we were coming home.” Dr Mizner loves her role as the internal medicine consultant for MFHC, focusing on autoimmune diseases, endocrinology, and HIV care. When not in the clinic, she can be found on the slopes and hiking trails.
BARBRA CORCORAN, BEHAVIORAL HEALTH COUNSELOR
Barbra Corcoran is a 23 year resident of the Western Slope of Colorado and has spent the last 16 in Glenwood Springs with her husband raising their four kids. Barb graduated with a B.A. from Canisius College in Buffalo, New York and from the University of Denver Graduate School of Social Work Western Colorado program in June 2015, receiving the Dean Catherine F. Alter award. She was also a Western Colorado Area Health Professions Scholar in 2015.
JOSH RUSK, MD
Site Medical Director
Josh joined the MFHC Edwards team in 2015. He completed family medicine residency in Fort Collins, CO after medical school at the University of Utah in Salt Lake City, Utah. He started his career as an engineer with a bioengineering degree from the University of Washington in Seattle, WA, but missed working more personally and directly with people. Rural medicine experiences in medical school, full-scope residency training, and travel early in his career helped Dr. Rusk recognize a desire to serve vulnerable populations similar to those he has witnessed in places like South America, Africa, and Southeast Asia. Dr. Rusk and his wife Brianne, a local science teacher, are heavily drawn to the mountains with their passion for the outdoors including skiing, biking, fishing, and chasing after their yellow lab named Canyon.
KENT PETRIE, MD
Kent Petrie earned his MD at the University of Virginia in 1976 and did his residency in family medicine at St. Paul Ramsey Hospital. In 2014 Dr. Petrie became the medical director for MFHC-Edwards, a clinic he started 20 years earlier as Eagle Care Clinic for the under-served patients of Eagle County. In 1996, he was recognized as Colorado Family Physician of the Year. He is an active elder at Eagle River Presbyterian Church where he teaches Sunday School. He travels on medical missions to Honduras and Tanzania, teaching and working in his area of special interest, maternal-child health care. He enjoys hiking, snowshoeing, amateur astronomy, model railroading, traveling and being a grandfather.
CAROL CONGER, CNM
Carol Conger is a native Coloradan. She was born and raised in Loveland, CO. She is a graduate of CU Health Science Center. Carol is a rare combination of Family Nurse Practitioner and Certified Nurse Midwife. Her passion is prenatal care, lactation and care of the expanding family. Carol is very active in the community facilitating advocacy for women, babies and their supportive partners. As a bilingual provider she embraces cultural diversity and brings a sense of humor and sincerity to all who enjoy her care.
DIANE PURSE, PNP
Diane Purse’s passion is working with infants, children and families. She graduated with her Pediatric Nurse Practitioner degree from State University of New York at Stony Brook in 1992. She has been a champion for our youngest, most vulnerable population in Eagle County for more than 15 years. Diane’s professional dedication focuses on childhood dental hygiene, preventative care and childhood literacy. When Diane isn’t tirelessly caring for the coming generations she enjoys reading, trail running and traveling.
DEB BURNS, ANP-C, GNP
Debra Burns was born a nurse at heart. From a very young age she would borrow her mother’s nursing books and teach the neighborhood kids all about health and a variety of other interesting anatomy topics on her back porch. She graduated Magna Cum Laude from the University of New Mexico with a BSN in 1986 and from the University Colorado Health Sciences Center with an MSN in 1992, with a dual Nurse Practitioner Adult/Geriatric specialty. Debra has an exquisite talent for making connections, listening and astutely diagnosing. Her specialty is internal medicine and care of the adult/geriatric client. Debra’s devotion to her complex population is stellar. Her adventuresome spirit took her to Alaska where she practiced Internal medicine from 1992-1996. She returned to Colorado and completed a pediatric fellowship and has been practicing in family/internal medicine in the Vail Valley since 1996. Debra’s varied interests include swimming, hiking, biking, skiing, and snowshoeing. She spends much of her time off on adventures with her two sons and husband. She also is extremely skilled at ping pong.
JOANNE TEIK, PA-C
Joanne Teik, PA-C, is a recent addition to Mountain Family Health Centers in our Edwards location. She is a transplant from Wisconsin and has been living in the Vail area on and off since 1986, and settled in Edwards after finishing her studies as a Physician Assistant in 2000 at Marquette University in Milwaukee, WI. Joanne has a good connection with the community, and looks forward to continuing to provide them with care and using her Spanish. In her off-time, she loves spending time with her two children, hiking, biking, rafting, SUPing, skiing, snowshoeing and all of the other outdoor activities that Colorado has to offer.

Originally from Maryland, Whitney moved to the Vail Valley in June 2015. Previously, Whitney worked in an integrated care setting at Barbara Davis Center for Childhood Diabetes and Children’s Hospital. Whitney earned her masters of social work degree in Health and Wellness from the University of Denver. Whitney enjoys skiing, rafting, traveling and hiking with her dogs during her free time.

JOHN LAWRENCE, PA
Mr. John Lawrence was born and raised in Los Alamos, NM. He received his BA in Chemistry from Simpson College in Indianola, IA; did graduate work at UNM and Drake University prior to his Physician Assistant studies at Red Rocks Community College in Lakewood, CO. A 2009 outstanding graduate award winner, he also received a Master of Medical Science degree from Saint Francis University in Loretto, PA. Mr. Lawrence’s background includes work experience in Nuclear, Environmental, and Geochemistry; recreation; and a 2 year stint as a Kindergarten teacher! Mr. Lawrence comes to Mountain Family Health after 5 years of Rural family medicine in the San Luis Valley of Colorado, and 1 and ½ years of rural outpatient pediatrics in Lander/Riverton, WY. He is very excited to be involved with the school based health clinic in Avon, as Pediatrics and Adolescent Medicine remain his passion. “ I am so privileged to work with our youth in medicine, as it provides a remarkable opportunity to not only make going to the doctor fun, but also to help instill life long health habits, and self-confidence in addressing their own healthcare needs.”
Mr. Lawrence speaks rudimentary Medical Spanish and looks forward to improving his conversational skills. When not in clinic his interests include: family, music, cooking, travel, camping, sports, and reading.
MELVIN F. BETANCOURT, LCSW
Melvin was born and raised in Guaynabo City, Puerto Rico. I graduated with a Bachelor’s degree in Psychology from La Universidad de Puerto Rico in 2002. He moved to Florida and joined the University of Central Florida, School of Social Work in 2004. and graduated in 2007 with a Master’s degree in Clinical Social Work. He then completed the requirements for license in the State of Florida in 2012. He was hired as a Behavioral Health Provider at Mountain Family Health Center and moved to Colorado in June/2016. He has experience working with children/families and adults with mental health/behavioral problems. He likes to travel, watch or play sports, hiking, outside activities and live music. He also likes working with people of all ages and backgrounds. He enjoys working in a team setting and making a difference in the community.

Sara earned her Master of Social Work degree from the University of Maryland, Baltimore. Originally from Portland, Oregon, Sara moved to the Roaring Fork Valley in 2010 and worked for Mind Springs Health providing crisis assessment, individual therapy and case management for adults and seniors. Sara believes in a solution-focused, strengths-based approach to social work, and enjoys being part of an integrated health care team. When not at work, Sara enjoys cooking, skiing and walking dogs at Aspen Animal Shelter.
CLAIRE CHAPPELL, PA-C
Claire joined Mountain Family Health Centers in May of 2015. She graduated in 2014 from Wichita State University with a Master of Physician Assistant degree that strongly emphasized primary care and allowed her to train in rural and under-served areas. She enjoys seeing patients of all ages and wants to help our community achieve lower rates of lifestyle-related diseases. Her background includes guiding on the rivers and trails of Colorado as well as owning her own business marketing outdoor recreation to visitors. She enjoys spending time with her husband and son and most weekends you’ll find her surfing river waves or working on a side project that promotes women in river sports. Claire sees patients primarily at the Basalt clinic and is working on learning a little bit more Spanish each and every day.
TRACIE McKINLEY, PA-C
Tracie graduated in 2011 from the University of St. Francis Physician Assistant program in Albuquerque, NM. She trained in under-served areas in New Mexico, Colorado, Georgia and Montana. Tracie moved to Shallowater, TX after graduation, and took a position with UMC Physician Network Services where she worked in family practice/urgent care settings at several of their rural health sites surrounding Lubbock. She has spent the last few years at the rural health clinic in Shallowater, focusing on preventative health care with an emphasis in patient education. Tracie is very excited to be a part of MFHC and looking forward to all that Colorado has to offer.
MATT PERCY, MD

Dr. Matt Percy grew up in Cortez, CO and after a brief stint in California for college, received a Bachelor of Science with honors in Biology from Stanford University. He received his MD from the University of Colorado School of Medicine and did a rotation at Mountain Family Health Centers during this time. This experience first opened his eyes to the beautiful Roaring Fork Valley and the wonderful patients and providers at MHFC. He completed residency in Family Medicine at the University of Utah in Salt Lake City and lives in the the valley along with his wife (a Pediatrician in Glenwood Springs) and his three children. He speaks moderate Spanish and is working on getting better. He is interested in the broad spectrum of care family medicine has to offer, and in particular prevention and pediatrics. When not at work, you can find him exploring the outdoors on foot, bike, or skis or travelling the world with his family.
CORRINNE JOHNSON, PA-C
Corrinne Johnson joined Mountain Family Health Centers in February of 2015. She comes to us from Salud Family Health Centers in Fort Collins, CO after spending a number of years working in community health. Corrinne was born and raised in north central Pennsylvania. She received her undergraduate degree from Penn State University. She moved to Colorado in 2002 after working her summers in Rocky Mountain National Park. Corrinne worked for Salud Family Health Centers in Estes Park for a few years before going back to school. She graduated from the Red Rocks Physician Assistant program in 2009 and earned a master’s degree from Saint Francis University. She moved to the Rifle area in July 2014 to be closer to the mountains. Her hobbies include hiking, biking, skiing, camping and backpacking with her husband. She enjoys seeing patients of all ages in order to help them achieve their health and wellness goals. She speaks a good amount of Spanish and is always working on getting better.
GREGORY SCOTT, DO
Dr. Gregory Scott is a new addition to MFHC serving as a family physician in our Rifle clinic. Dr. Scott is an osteopathic physician board certified in Family Practice who graduated from Nova Southeastern University in Florida. Prior to moving to Colorado, Dr. Scott was practicing in Tampa, Florida doing a combination of scientific research and family medicine with an emphasis on geriatric medicine. Dr. Scott lives in Edwards with his wife and young son and is enjoying exploring all the natural outdoor activities Colorado has to offer. When not at work Dr. Scott and his family enjoy skiing, biking and traveling.
NATASHA ELLWOOD, PA-C
Natasha has been seeing patients of all ages at Mountain Family Health Centers since April of 2015. She graduated in 2013 from Midwestern University’s Physician Assistant program in Phoenix, Arizona. Before coming to Mountain Family she spent a year working for Aspen Orthopaedics in Glenwood Springs. With her background in sports medicine she enjoys seeing patients with musculoskeletal complaints, but is embracing family medicine, focusing on preventative care and management of chronic disease. Born and raised in Colorado, she enjoys spending time outside running, camping and hiking, as well as traveling and knitting.
GARRY MILLARD, DDS
Dr. Garry Millard, DDS is our Dental Director. He attended Montana State University, the Dental School at the University of Minnesota, and did his Periodontal Residency in Bethesda, MD. Dr. Millard is a dedicated family man. He and his wife Stacey have two natural and two adopted children. He is also passionate about improving dental access for underserved populations. Dr. Millard helped develop the Colorado SMILES dental project, which he will launch at four new school-based sites in 2017. He has also been instrumental in developing our new Basalt Mobile Dental Program, which also launches in 2017. Dr. Millard loves hiking, golfing, and skiing, and is delighted to be working for Mountain Family. After 10 years as a Navy Dentist, and 15 years in private practice, he is happy to be making a difference for the patients that face significant barriers to obtaining affordable dental care.
LAUREN ROPER, DDS
Dr. Roper is a transplant from Texas and moved to the Western Slope to enjoy all of the outdoor activities. She earned her dental degree at the University of Texas Health Science Center in San Antonio and before joining Mountain Family Health Centers, she worked in private practice as an associate in Grand Junction. Dr. Roper and her husband enjoy camping, climbing, biking, rafting, skiing, and playing with their two dogs.
STEPHEN GLASS, DDS
Stephen joined the Mountain family dental team in Rifle in 2015. Dr. Glass became interested in dentistry through a combined Missions of Mercy and Remote Area Medical clinic in the small town of Wise, Virginia. Ever since, he has been passionate about not only dentistry, but also working closely with the community. He attended dental school at Virginia Commonwealth University and upon graduation, he and his wife Kati drove west. Along with their dog Oso, they enjoy being in the mountains climbing, skiing, and biking.
GARY SCHREINER, PHD

Dr. Schreiner is a native Coloradoan who grew up in the Boulder area and eventually relocated to the Western Slope where it was a little slower paced. He completed his Doctorate in Marriage and Family Therapy at Texas Tech University and has worked in the medical field on and off for about 30 years. He has had a private practice, directed several programs with delinquent adolescents, worked in psychiatric hospital settings, medical clinics, and contracted out to the military. He provided counseling to servicemen and their families throughout the world. In his ‘prior’ life, he was a school counselor and coach. Although he left the education system, he still found time to coach youth football and baseball for over a decade. He currently enjoys spending time with his beautiful wife, his German Shepherd and he likes to ski, play golf, scuba dive and traveling the world!

Oyen Hoffman holds a Master’s Degree in Psychology with a Marriage & Family Therapy emphasis from Chapman University in Southern California as well as two Licenses to practice in Colorado. He is a Licensed Marriage & Family Therapist and Licensed Addiction Counselor. Oyen is a Nationally Recognized speaker and trainer with expertise in many behavioral health related areas and has had the distinct honor of speaking at Washington D.C. regarding family systems and PTSD, as well as several conferences nationwide. Oyen is also a published author and dedicated family man. Oyen is passionate about serving Mountain Family Health patients and proud to be a member of the Mountain Family Health Center team.
