Health Care Providers

The Providers at Mountain Family Health Centers bring years of specialized training and experience to our practice. Our nurses and doctors actively participate in continuing education programs and continually strive to stay current with health trends and practices. In addition, they are involved in the communities we serve by conducting health fairs and are involved with various other non profit organizations. Our Community Health Center is one of the most technical in the state and our staff utilizes Electronic Medical Records to monitor and track our patient’s health and well being. Many of our providers and staff are fluent in Spanish, SE HABLA ESPANOL.

Black Hawk Clinic

Kelli Eberhardt, PA-C

Kelli is a native of Western Colorado and was raised in the Roaring Fork Valley. She has been working for Mountain Family Health Centers in Black Hawk since September of 2011. Kelli graduated with a Bachelors of Science in Human Nutrition from Colorado State University in Fort Collins, Colorado. She then attended Red Rocks Physician Assistant program in Lakewood, Colorado and obtained her Masters degree in Medical science from St. Francis University in Loretta, Pennsylvania. Kelli has a special interest in women’s healthcare and preventative medicine. When not at work, Kelli enjoys spending time outdoors with her husband and two young children.

Carol Holdcroft, FNP

Carol  has been a nurse practitioner for 30 years. She has  lived in the Nederland area for 25 years with her husband and dogs and cats. Although she loves seeing patients of all ages, she particularly enjoys taking care of older patients.  When not working, Carol enjoys skiing, bicycling, gardening and riding her Harley motorcycle.

 

Glenwood Springs Clinic

Ken Davis, PA-C

After obtaining his Bachelor of Science at the University of Wisconsin, Ken volunteered for 2 years in Lesotho, South Africa with the United States Peace Corps developing and managing community based public health programs promoting maternal and child health and nutrition. Ken returned to University of Wisconsin and completed the Physician Assistant program with Dean’s Honors. Before joining Mountain Family Health Center in 2003, Ken provided a full spectrum of primary care in a non-profit Navajo rural health center in Arizona as a National Health Service Corp Scholar. Ken speaks basic Spanish and has traveled to many countries including Lesotho, South Africa; Namibia, Botswana, Zimbabwe, Egypt and Italy. Ken has won awards for his work with detection and treatment of Diabetes. Ken is active with local Community Health Fairs, cardiovascular health outreach and with the American Diabetes Association, Por Tu Familia Program which offers free Latino counseling, education and cardiac risk screening. “I look forward to providing a wholistic and integrated approach to healthcare, with a particular interest in cardiovascular health in underserved populations.”

Chris Tonozzi, MD

Dr. Tonozzi has been with Mountain Family Health Center since 1999. He earned his M.D. at Colorado School of Medicine and practiced at the University of Colorado Department of Family Medicine. Dr. Tonozzi is fluent in Spanish and is responsible for making our clinic one of the leading Community Health Clinics in the state of Colorado. Mountain Family Health Center prides Dr. Tonozzi’s efforts in adopting Electronic Health Records and technical computerization of our clinic. When Dr. Tonozzi is not in the office you’ll find him on the slopes and rivers enjoying time with his family.

Amy Brown, MD

Dr. Brown joined Mountain Family Health Center in 2004 after completing her residency at Rose Family Medicine in Denver, CO. Amy received her MD from Temple University in Philadelphia, PA and a BA in Italian from Vassar College. As a result of having grown up while living abroad in S. Africa, Brazil and Chile, Amy is fluent in Spanish and this contributes to her special interest in serving a multicultural population. Amy is very interested in integrative medicine and is eager to complete a fellowship in this field. She spends most of her very little free time in the pursuit of spiritual evolution.

Jenny Lang-Burns, FNP, CNM

 Jenny has been with Mountain Family Health Center since 2000 after completing graduating from the University of Colorado’s Family Nurse Practitioner program. Previously Jenny had completed her Certified Nursing degree at the University of Rochester nursing program and finished her Certified Nurse Midwife degree from the University of Pennsylvania. Jenny had the opportunity to serve several years in the United States Peace Corp in Yemen practicing at health clinics in this poverty stricken area. It was during her stint at an Ecumenical Mission in Guatemala that Jenny found her passion working with mothers and children. Jenny went on to serve at a Free Standing Birth Center in the Bronx, New York developing her Midwife birthing skills. Jenny moved to Glenwood Springs, Colorado in 1995 and now enjoys practicing part-time at Mountain Family Health Center and also contributes at Mountain Family Developmental Center. Jenny enjoys spending time with her husband and two daughters experiencing the wonders of the Colorado Mountains.

Susan Orcutt, FNP

 Mountain Family Health Center welcomed Susan Orcutt to our staff in July of 2008. Susan graduated from the University of Colorado Health Sciences Center’s School of Nursing in Denver, CO in May of 2007 with a Master of Science in Nursing, Family Nurse Practitioner degree. She received her Bachelor of Science in Nursing from the University of Colorado Health Sciences Center’s School of Nursing, graduating Magna Cum Laude in 2003, following completion of her pre-nursing requirements at University of Colorado in Boulder. Susan brings valuable experience from Grand River Medical Center in Rifle, University of Colorado Hospital in Denver, and Boulder Valley Women’s Health Center. “I’m looking forward to promoting health and wellness in our community,” said Susan Susan is a Colorado native and when she is not caring for patients in the clinic she can be found enjoying time outdoors with her family skiing and backpacking in the mountains of Colorado and Utah.

Gail Mizner, MD

Dr. Mizner earned her BA in French Literature at Amherst College and her MD at the Yale University School of Medicine. She returned home to Colorado to do her residency in internal medicine at the University of Colorado Health Sciences Center and then stayed on at UCHSC as a faculty member in the General Internal Medicine Division and the Head of Physician Education for the AIDS Education and Training Center. In 1995, Dr. Mizner and her husband and two sons moved to Ely, Nevada where she enjoyed practicing as the only internist in an extremely rural and isolated setting. In 1997, the family moved again to Northern Chile. After becoming fluent in Spanish and taking 6 exams to revalidate her medical degree, she practiced internal medicine and ran HIV programs in two different Chilean public hospitals. Since returning to the States in 2004, Dr. Mizner has been practicing internal medicine and HIV care in Prescott, AZ. She says that “moving to the Roaring Fork Valley feels like coming home” and that as the first internist at Mountain Family, she is “looking forward to participating in the care of the clinic’s most medically complex adult patients.” When not in the clinic, she can be found on the slopes and hiking trails with her family.

Sandy Deveny, MD

Dr. Deveny comes to Mountain Family Health Center from Akron, Ohio where she practiced at the MEDGroup as a staff physician specializing in Urgent Care and Occupational Medicine after 12 years in emergency medicine. Dr. Deveny also worked with B.S. Bonyo, D.O. at his inner city office and was instrumental in developing a Health Center in Kenya with Dr. Bonyo. She has also volunteered at the Open M free clinic in Akron. Sandy’s children now attend Colorado Rocky Mountain School in Carbondale which allowed her to move to western Colorado and practice part-time at MFHC. Dr. Deveny graduated from The Medical College of Ohio at Toledo and earned her residency from St. Vincent Medical Center / The Toledo Hospital with Board certification in Emergency Medicine. Dr. Deveny is certified to practice in both Ohio and Colorado. She has also traveled and practiced medicine in Peru as well as co-founding Care Kenya and practicing at the medical clinic in Kenya, Africa. Sandy enjoys spending time with her family and dogs on the western slope and enjoys running, hiking, biking, skiing, photography and cooking.

Sandra Plybon, MD

Sandra is from Slovakia, she received her MD degree from the Charles University in Prague, Czech Republic.  She finished her residency at the University of Wyoming Family Medicine Program in Cheyenne.  It is the opportunity for long-term interactions with families that still exists in the practice of Family Medicine that drew her to this specialty.  The idea of whole and complete wellness is where her focus lies, and her aim is for her patients to have healthy life experiences.  Having traveled extensively throughout Europe and the U.S. she has enjoyed many different cultures.  She intends to travel to Vietnam, Laos and Cambodia next.  She enjoys hiking both in her homeland and in the Rocky Mountains.  She jogs most days and swims often.

Jeff Wuerker, MD

A long-time resident in Colorado. Dr. Wuerker started his medical career as an EMT/Paramedic which led him to go back to college at the CU-Boulder and then on to receiving a medical degree at UCHSC in Denver. He trained in Emergency Medicine at Emory University in Atlanta, GA.
Has been a resident in Glenwood area since 2001, and previous practiced medicine at Valley View Hospital as an ER physician for 10 years. He is proud to have joined Mountain Family to provide acute care to the people of Garfield County.
Outside interests include skiing, rafting, hiking and photography.

Basalt Clinic

Rachel Menke, BSN, MSN, FNP

Rachel is a native of New Mexico. She is a  graduate of the University of New Mexico earning her BSN, MSN and Family Nurse Practitioner degrees. Rachel has a professional interest and experience in women’s health and chronic disease management with an emphasis on helping people be an active part of their health care. Her professional philosophy is that even with chronic health problems you can be healthy and active.  Rachel helped establish the UNM Center for Reproductive Health in 2007 . She is married and her husband is a native of Carbondale, Colorado. She fell in love with the area six years ago and has been returning frequently. She is very fortunate and happy to be moving to the area to help care for her disabled father-in-law and to enjoy the great outdoors of Colorado. Her hobbies include yoga, biking, running, hiking and skiing. She learned to downhill and cross country ski in Aspen/Carbondale area. Rachel has  three dogs, who are spoiled rotten and are also happy to be moving to Colorado!

Rifle Clinic

 Nicole Stalter, MSN, ANP, FNP-C

Nicole was raised and lived in Iowa before moving to the Glenwood Springs area to be closer to family. She has been practicing as a Family Nurse Practitioner for twelve years and possesses a breadth of experience in Family Practice and Internal Medicine. She is a graduate of Drake University located in Des Moines, Iowa, where she earned her Master of Science degree in Nursing with a Family Nurse Practitioner focus. Nicole earned her Bachelor of Science degree in Nursing in 1998 and also holds a Diploma in Nursing from Mercy College of Health Sciences, also located in Des Moines.

Diabetes prevention and treatment is a passion for Nicole. Her approach to patient care includes several aspects of their lives; involving their physical, psychological and spiritual well-being.

She finds enjoyment in spending time with her husband and children, attending church, participating in outdoor activities, and continually learning about history. Nicole looks forward to meeting and caring for the patients of Mountain Family Health Center and helping them find their way to health and well-being!

Dr. Garry Millard, D.D.S.

Dr. Millard is the Dental Director for Mountain Family Health Centers.  He is a native of Montana.  He received his Dental Degree from the University of Minnesota, and served as a Dental Officer in the US Navy for 6 years.  He was selected for specialty training in periodontics, and attained board certification in 1995.  He practiced as a periodontist in Northern California for 15 years before joining our team.  He enjoys mountain biking, snowboarding, golfing, hiking, and raising his four kids.  Dr. Millard has two natural children, and has adopted two little girls.

Dr. Patricio Gonzalez, D.D.S.

Dr. Patricio Gonzalez earned his DDS from the University of Missouri Kansas City School of Dentistry in the spring of 2001. Prior to dental school he completed a BS in Biology with a minor in Spanish Literature from Truman State University in Kirksville, Missouri. Dr. Gonzalez’ professional experience includes over ten years of patient treatment in Military, Public Health and Private practice settings, as well as overseas volunteer work.  Dr. Gonzalez is fluent in Spanish and looks forward to serving the dental needs of the multicultural population of the Colorado and Roaring Fork river valleys in Garfield County.

Kari Brennan, RDH

Kari was born in Meeker, Colorado.  She graduated from Meeker High School in 2003.Kari attended Colorado Northwestern Community College in Rangely, Colorado where she graduated from the Dental Hygiene Program in 2006.  Temporarily, Kari worked as a Hygienist in Grand Junction for three months right out of school.  Then, July of 2006, Kari started working at Castle Peak Dental in Eagle, CO. and continued there for 5 ½ years before starting at Mountain Family Health Centers-Dental.  Kari has been working for M.F.H.C. since opening in June 2011.  July 2007, Kari married a rancher from Meeker and now they both live and work on the Brennan family ranch.  Kari enjoys ranching, hunting, riding horses, team roping and spending time with family… and of course Dental Hygiene!!
