Mountain Family Health Centers (MFHC) are in Glenwood Springs, Basalt, Edwards and Rifle, Colorado. Each community health center is as unique as the town in which it is located, but all share the same mission:

“To provide high quality, integrated primary medical, behavioral, and dental health care in the communities we serve, with special consideration for the medically under-served, regardless of ability to pay.

We are pleased to have the opportunity to provide your healthcare at Mountain Family Health Centers! Whether you need a routine physical exam, have the flu, need help managing your diabetes or depression, or urgent care, you can trust us for high quality service regardless if you have insurance or not. We accept most private insurances, Medicaid and Medicare, and offer sliding scale programs and other assistance programs for any person who is uninsured. 

The MFHC Board of Directors have unanimously decided to support Colorado’s Tobacco Tax Increase, which will be Initiative 72 on the November Colorado ballot. The Initiative would raise the state tax on cigarettes by $1.75 per pack and proceeds will be reinvested in important tobacco cessation and prevention programs, Veterans access programs, Community Health Center expansions, Community Mental Health expansions, and Healthcare Workforce expansions.
