Mountain Family Health Centers (MFHC) are in Black Hawk, Glenwood Springs, Basalt, and Rifle Colorado. Each community health center is as unique as the town in which it is located, but all share the same mission:
“To provide high quality primary health care in the communities we serve, with special consideration for the medically under-served and uninsured. Our board-certified physicians, physician assistants, and family nurse practitioners offer you the full range of primary health care services. We welcome any person in need of care, regardless of ability to pay. We accept most private insurances, Medicaid and Medicare, and offer sliding scale programs and other assistance programs for any person who is uninsured.”
