Mountain Family Health Centers (MFHC) are in Glenwood Springs and Black Hawk. Each community health center is as unique as the town in which it is located, but all share the same mission:
To provide high quality primary health care in the communities we serve, with special consideration for the medically under-served and uninsured.
Our board-certified physicians, physician assistants, and family nurse practitioners offer you the full range of primary health care services.
We welcome any person in need of care, regardless of ability to pay. We accept most private insurances, Medicaid and Medicare, and offer sliding scale programs and other assistance programs for any person who is uninsured.
Mountain Family Health Centers uses the Colorado Immunization Information System
(also known as CIIS).
CIIS is a database for immunizations. All immunization (adults & children) records from our health center are sent to this state database.
If you would like to remove your record or your child’s record from this database, you must obtain an opt-out form from our health center and send the form to CIIS.
